Membership Application

We welcome new members!

Annual dues are $45.00 per household or $40 for an individual.  $5.00 will go to the BCHA legal fund and it is not tax deductible.  The balance of your membership is tax deductible.  Memberships are annual and renew on January 1st.  If you join after October 1 your membership is good for the entire next year.

 

If you wish to join by mail click on Download Application to download the  form.  Mail the completed form and a check, payable to BCHCAZ to the address indicated on the form.
